All student athletes in grades 7-12 in the Southern York County School District need to have submitted a completed PIAA Physical prior to the start of the season.
All physical forms shall be submitted to the Athletic Trainer, Kallie Heiges Fancher, in the High School.
PIAA Physical Forms can be downloaded at the bottom of the page. Each student athlete must have completed & submitted Sections 1-7 each school year prior to the start of their first sports season. If an athlete is participating in more than one sport season, Section 8 needs to be completed by their parent/guardian. When completing Section 8, if there are "Yes" responses to any of the Supplemental Health History questions, Section 9 must be completed by a physician.
Winter Sports Physicals are being held at ALL OSS Health locations (Powder Mill, West York, and Hanover) on the following dates from 6-8pm. You must call OSS at 717-848-4800 to schedule an appointment. $15 cash, credit card, or check.
Powder Mill: Wednesday, November 2 & Wednesday, November 9
West York: Tuesday, November 1 & Tuesday, November 8
Hanover: Tuesday, November 1 & Tuesday, November 8
First day to turn in Winter Sports Physicals: Friday, October 7th, 2022
Physical paperwork & ImPACT baseline testing must be completed by Friday, November 11th, 2022
Physicals will be held for all Student Athletes grades 7-12 on Tuesday, June 14 and Wednesday, July 27 in the Susquehannock Middle School gymnasium from 6-8pm. They will cost $15 payable by cash or check. Physicals will be first come, first serve and no appointments necessary.
