Physical Information

  • All student athletes in grades 7-12 in the Southern York County School District need to have submitted a completed PIAA Physical prior to the start of the season.

    PIAA Sections 5-6 are due in the athletic training room to Trainer Kallie no later than: Monday August 5th. You can scan and email them to kallie.heiges@sycsd.org or turn them into the high school main office to put in Trainer Kallie's mailbox.

     NEW, we are not using ImPACT anymore so baseline ImPACT tests are not required for tryouts. 

    **NEW** We are using a new system for Physicals, RankOne.  Below is the instructions on how to complete PIAA Sections 1-4 online.

    Instructions for completing PIAA Paperwork Sections 1-4 and Eligibility Contract on RankOne:

    • Go to sycsd.rankone.com
    • Click “Proceed to online forms”
    • Login, Create new account, or Continue as a guest
      • If logging in or creating a new account:
        • Search for your student using Last Name and District assigned Student ID number
        • “Start Forms”
        • “View” next to their name
        • Click on each of the “Section” links that are in blue and complete each
      • If continuing as a guest:
        • Click on each of the “Section” links that are in blue and you will be required to enter your students name and District assigned Student ID number at the top of each form

     

    PIAA Physical Forms can be downloaded at the bottom of the page. Each student athlete must have completed their online portion for Sections 1-4 on RankOne & submitted Sections 5-6 to Trainer Kallie prior to the start of their first sports season. If an athlete is participating in more than one sport season, Section 7 needs to be completed by their parent/guardian. When completing Section 7, if there are "Yes" responses to any of the Supplemental Health History questions, Section 8 must be completed by a physician.

     

     

    Fall Physicals:

    Physicals will be held for all Student Athletes grades 7-12 on Wednesday, June 12 and Wednesday, July 17 in the Shrewsbury Elementary School gymnasium from 6-8 pm.  They will cost $15 payable by cash or check.  Physicals will be first come, first serve and no appointments necessary. 

     

    First day to turn in Fall Sport Physicals: May 1, 2024

    Physical paperwork must be completed by Monday August 5, 2024

    First day of Football Heat Acclimation: Monday August 5, 2024

    First day of Fall Sports: Monday August 12, 2024
