Physical Information

  • PHYSICAL INFORMATION: (Updated 11/6/2019)
    Please read carefully. This procedure affects all student athletes in grades 7-12 in the Southern York County School District. If your son or daughter is planning on playing a fall, winter or spring sport, the following dates for physicals are listed below.

    All student athletes are encouraged to get their physical on one of the dates below.

    * ALL PHYSICAL FORMS ARE TO BE RETURNED TO THE ATHLETIC TRAINERS (BETH PARRISH AND KALLIE HEIGES) IN THE HIGH SCHOOL, NOT THE HEAD COACH. *

    PIAA Physical Forms can be downloaded at the bottom of this page.   

     

    WINTER SPORTS:

    Winter physicals must be dated on or after June 1, 2019. 

    Physicals and Impact are due to us in the training room no later than November 11th, 2019.  You can start turning them in starting October 7th, 2019. 

    *Please note, if your physical is dated before October 7th, 2019, you will need to complete PIAA Section 7.  

    If you still need a physical, OSS will be hosting open physicals from 6-7pm, $15.00 per individual athlete, on the following dates:

    Wed Nov 6 from 6-7pm

    Wed Nov 13 from 6-7pm 

     

     

    Fall Sports:

    Fall physicals must be dated on or after June 1, 2019

    Physicals and Impact are due to us in the athletic training room no later than Monday, July 29, 2019

    The first day of football heat acclimation is Monday August 5, 2019. The first day of fall sports is Monday August 12, 2019.


    PHYSICAL DATES:

    MondayJune 102019 and ThursdayJuly 25, 2019 

    Location: Susquehannock High School in the Main Gym

    Time: 6:00 pm - 8:00 pm


    COST:
         $15.00 per individual athlete.

        Please make checks payable to the Southern York County School District.

     

    * If you cannot make the Susquehannock physical dates, OSS will be hosting open physicals at the Powder Mill location from 6-7pm, $15.00 per individual athlete, on the following dates:

    Wednesday July 31

    Wednesday Aug 7 

     

    Please Note: If you are unable to attend sports physicals at school, you will need to make arrangements for your own physical with your family physician.
