All student athletes in grades 7-12 in the Southern York County School District need to have submitted a completed PIAA Physical prior to the start of the season.
All physical forms shall be submitted to the Athletic Trainer, Kallie Fancher, in the High School.
PIAA Physical Forms can be downloaded at the bottom of the page. Each student athlete must have completed & submitted Sections 1-7 each school year prior to the start of their first sports season. If an athlete is participating in more than one sport season, Section 8 needs to be completed by their parent/guardian. When completing Section 8, if there are "Yes" responses to any of the Supplemental Health History questions, Section 9 must be completed by a physician.
Physicals will be held for all Student Athletes grades 7-12 on Tuesday, June 14 and Wednesday, July 27 in the Susquehannock Middle School gymnasium from 6-8pm. They will cost $15 payable by cash or check. Physicals will be first come, first serve and no appointments necessary.
First day to turn in Fall Sport Physicals: Wednesday June 1, 2022
Physical paperwork and ImPACT baseline testing must be completed by Monday August 1, 2022
First day of Football Heat Acclimation: Monday August 8, 2022
First day of Fall Sports: Monday August 15, 2022
